In the context of an increasingly aging population and increasing rates of chronic diseases, such as hypertension and diabetes in the elderly, both conditions require continuous monitoring for effective management, reduce the risk of complications. However, currently, many elderly people have difficulty moving to medical facilities, due to reduced mobility or lack of transportation. Meanwhile, medical facilities are not able to meet the regular monitoring needs of patients.

Therefore, promoting the development and deployment of remote monitoring technologies is an important step and should be a top priority in current health policy. This not only helps reduce the burden on the health system but also increases access to quality health services for the elderly, while minimizing the risk of infection in the hospital environment.

The article was ordered: Remote patient monitoring solution for two common diseases of the elderly in the community: hypertension and diabetes (monitor blood pressure and blood sugar at home).

Ordered problem: Platform solution for sharing electronic medical records for general hospitals in Ho Chi Minh City

Selected models and solutions will receive financial support and incubation to complete and introduce to departments and localities for practical pilot implementation. In addition, project implementation teams will be trained, mentored, and incubated to perfect solutions, products, business models, and etc. to meet application needs in the public sector.

Projects entering the incubation round have the opportunity to receive a maximum support package of 400 million VND and be connected to beneficiary units as a basis for practical implementation
